James Avery was suffering from several health issues for years before his death at the end of last month. TMZ has obtained the death certificate for the Fresh Prince actor, which shows that Avery had severe coronary heart disease, end-stage kidney disease and type two diabetes.

Avery was sixty-eight when he passed on December 31st, about seven weeks after undergoing bypass surgery following a heart attack. The official cause of death was cardiorespiratory arrest. Avery was cremated, and his remains were scattered at sea off the coast of Los Angeles.
